This nineteenth century tomb is situated in the churchyard of Hafod Church (NPRN 743). It belongs to Thomas Johnes, renowned owner of Hafod (from 1780), who was responsible for rebuilding the church in 1803. He died at Dawlish, Devon in April 1819, but was buried here. Its enclosing rails and wall are Grade II listed.

The walls are constructed of rubble stone, with short side walls linking to railings. The railings are of the conical lance head type, on chamfered low stone walls with flat coping.
